Artistically necessary. Euphoria is messy and extreme because teen pain is messy and extreme; sanitizing it would be the real dishonesty.
"Messy" doesn't require lingering close-ups of nude teenagers or stylizing addiction like a perfume ad. You can depict pain honestly without aestheticizing it for the audience's pleasure.
